Meningitis B is not accepted, as it does not fulfill the State of Texas’ legal requirement for admission. Unfortunately, we cannot accept Men B immunizations under any circumstances.

Acceptable evidence of vaccination or receiving a booster dose includes:

• The signature or stamp of a physician or his/her designee, or public health personnel on a form that shows the month, day, and year the vaccination dose or booster was administered
• An official immunization record generated from a state or local health authority
• An official record received from school officials, including a record from another state

The following San Jacinto College students are not required to submit evidence of receiving the vaccination against bacterial meningitis or evidence of receiving a booster dose:

• Students who are 22 years of age or older
• Students who enrolled during the Fall 2011 term or earlier and who have maintained continual Fall and Spring term enrollment as long as they remain San Jac students. (Note: the exception does not apply once a San Jac student transfers to another college.)
• Students who are enrolled only in Continuing and Professional Development courses or corporate training programs that are less than 360 contact hours that are not linked to Credit classes. Contact Us for more information. 

Following are the types of waivers. If requesting a waiver, then you won't need a meningitis vaccination, but you will need to fill out the bacterial meningitis form and attach your waiver documentation. 

• Distance Learning (DL) waiver: If you're enrolled only in distance learning classes that are 100 percent online.
• Texas (TX) waiver: If you're presenting a conscientious exemption stating you decline the vaccination for reasons of conscience, including religious belief. Access the conscientious exemption form.
• Health waiver: You'll need to present an affidavit signed by a licensed physician stating that the vaccination would injure your health.

Instructions:

• Complete the bacterial meningitis form in step one above, then attach your documents. 
• Make sure your name, birth date, student ID number (G number) and all other information are displayed correctly on the waivers. Your G number is 8 digits. Also make sure waivers are signed and dated so we can accept them.
• DL waiver: Check the second box on the form in step one  that says I am declaring a temporary waiver. Then select the semester and year you want the waiver applied. Note the hold will never be deleted with this option. The hold date will simply be adjusted to reflect the semester you are attending. 
• TX waiver: After you fill out your birth date on the form, if you are 18 or older, a third box will pop us that says I am 18 years of age or older and am declaring an exemption for reasons of conscience. Check that box. Fill out the conscientious exemption form. Click the print button. Print it out. Sign and date the form. Upload it where it says attach a file. 
  
  Note the hold will never be deleted with the TX waiver option. The hold date will simply be adjusted to two years from the signature date. 
• Health waiver: Check the last box on the form. Upload your affidavit signed by a licensed physician.
